Essay

In dissertation we have characterized heart rhythm and conductivity disorders in patients with type 2 diabetes mellitus and heart ischemic disease (HID). Object: heart ischemic disease in patients with type 2 diabetes mellitus (DM). The aim of work is to establish the natural development of heart rhythm and conductivity disorders in patients with type 2 DM with HID and to work out the methods of their prophylaxis and treatment. Methods and apparatus: clinical, biochemical, instrumental; electrocardiograph 6 NEK-401, apparatus EPICARDIA, ultrasonic doppler-echocardiograph. Theoretical and practical results: on the basis of complex examination (ECG, holter monitoring of ECG, Doppler-echography, biochemical methods) the natural beginning of heart rhythm and conductivity disorders were studied in patients with type 2 DM and HID. The peculiarities of hemodynamics changes were revealed, the scheme of using retard form of verapamil hydrochloride in complex with mildronate and preparations of alpha-lipoic acidwas developed and introduced into the treatment of supraventricular arrhythmias. The new: relation with clinical duration of extrasistoly and instable signs of glycemia, arterial hypertension, chronic diabetic microvascular complications, dyslipidemia, rheologic and electrolit blood changes in patients with type 2 DM and HID was established. It has been discovered that effectiveness of treatment with verapamil, mildronate, and alpha-lipoic acid is connected with influence on pathogenetic factors of the appearance of supraventricular arrhythmias. The level of approach: the results can be used in the treatment of the patients with type 2 DM and HID and disorders of heart rhythm. Field of application: endocrinology, cardiology, therapy.
