Litvinova N. Surgical treatment of atherosclerotic occlusion and stenosis of brachiocephal arteries in patients with coexisting ischemic heart disease.

The dissertation is devoted to surgical treatment of atherosclerotic occlusion and stenosis of brachiocephal arteries in patients with coexisting ischemic heart disease. Data obtained from investigation of 205 patients with chronic brain circulation insufficiency (HBCI) underwent operative treatment, 135 (65,9%) from them suffered from ischemic heart disease (IHD). Elaborated in our clinic 3staged algorithm of patients investigation with chronic cerebral ischemia has been used for diagnostic. Surgical reconstruction is indicated to patients with HBCI I-IV stage and 75% and more stenosis, in patients with summary stenosis of 45%, in cases if stenosis caused by atheroslerosis with ulcer, thrombosis or calcium. Carried out by us new operative allowed us to reduce complications in postoperative period. Patients with IHD underwent pre and postoperative treatment. Elaborated surgical tactic allowed us to obtain good results in 92,7% patients and in 84,6% in late follow-up.
