Essay

The present study aimed to improve the technique of delivery assistance in IUGR through determination of fetal state and growth parameters with highest diagnostic value and elaboration of the main principles of prenatal monitoring.Performed investigations have shown that the efficiency of ultrasound evaluation of fetal development is considerably improved by extended fetometry with calculation of head and abdominal circumference and fetometry indices. The main parameter of the cardiotocogram which responds to changes of conditions in which the fetus develops in IUGR is the type of heart rate variability. Fetal motility and amniotic fluid volume are other two sensitive indicators of fetal condition in pregnancies complicated by IUGR. Doppler evaluation of blood flow in uterine and umbilical cord arteries and in placental bed vessels is also significant for diagnosis and prognosis. Obtained results provided the basis for elaboration and introduction into clinical practice in obstetric establishments of technology of prenatal monitoring in IUGR which permits to obtain actual information concerning condition of the fetus and take appropriate therapeutic measures, determine best time and approach to the management of delivery and decrease 4-fold perinatal mortality.
