Essay

The thesis was aimed at the improvement of the results of acute necrotizing pancreatitis surgical treatment for acute necrotising pancreatitis. The complex clinico-morphological, laboratory, instrumental examinations and treatment of 192 patients contracted severe necrotizing pancreatitis were performed. The data obtained were used to develop and substantiate the algorithm complex treatment determining the taking turns, optimal terms and a list of conservative and surgical measurements depending on the stage and clinical trial of severe necrotizing pancreatitis. It was also developed the modified technique of "open abdominal" surgery - dynamic omentopancreatostomy (DOPS). The advantage of this method as compared with "traditional" ones was proved. Using of DOPS in treatment of patients with severe necrotizing pancreatitis enabled to improve the clinical course of disease, decrease the numbers of fatal postnecrotic complications and lethality from 51,7% to 29,9%. Besides, the positive influence of intravenous laser blood irradiation on these results and its immunomodulating effect were shown.
