Object of the study: hypertension disease and pregnancy. Purpose of the study: decline of frequency of development of complications of pregnancy, disorders in the system «uterune-placentae-fetus» at hypertension disease on the basis of study of changes of metabolism, central and uterune-placentae-foetal haemodynamics, state of the fetal CNS and development patogenic motivated principles of their medical treatment. Methods of the study: clinical instrumentation, biochemical, echocardiography methods, dopplerometry, magnetic-resonance imaging, methods of mathematical statistics. Practical significance of the obtained results: Consists in development the complex of functional, clinical and biochemical criteria which allow to estimate the changes of central and uterune-placentae-foetal haemodynamics, and also metabolism in the system «uterune-placentae-fetus» at women with hypertension disease for the prognosis of complications of pregnancy and control of efficacy of remedial preventive measures. Accordingto finding tactic to manage pregnancy and delivery have been worked out: purposeful antihypertensive therapy taking into account the type of central haemodynamics, normalization of metabolic disorders, correction of haemodynamics breaking in the system «uterune-placentae-fetus». Introduction in practice of objective diagnostic criteria, prognostic values of factors of risk, perfections of methods of medical treatment of pregnant with hypertension disease, correct choice of methods and terms on the delivery allowed to lower frequency of complications in pregnant at hypertension disease, and also frequency of origin of heavy complications at newborn in an early neonatal period. Novelty of the obtained results: On the base of the clinical instrumentation methods of research the role and significance of metabolic and haemodynamics breaking in pathogenesis complications of pregnancy and disorders of the state of fetus at hypertension disease have been studied and on this base patogenic motivated principles of their correction have been designed. Intercommunication between the state of circulation of blood in the system «uterune-placentae-fetus» is proved, by the indexes of central haemodynamics and metabolic disorders. The prognostic criteria of indexes of metabolism are set, central and uterune-placentae-foetal haemodynamics for determination of development of complications of pregnancy and disorders of the state of fetus in inspected pregnant. Usage of magnetic-resonance imaging allowed to diagnose hypoxic fetal CNS injuries with a high precision for the first time. Taking into account the selected criteria of character of metabolic disorders, changes of central haemodynamics, haemodynamics breaking in the system «uterune-placentae-fetus» and variants of hypoxic fetal CNS injuries complex medical treatment have been worked out, and also tactic to manage pregnancy and delivery at hypertension disease, which are directed on warning of development of heavy complications of pregnancy. Introduction: The findings of the research were introduced to the work of medical establishments of the South railway, Kharkiv Regional Clinical Hospital, Clinical Maternity Hospital No. 2, No. 3 (Kharkiv).
