Dvoyashkina Y. Immune disorders at peptic ulcer associated with Helicobacter pylori and their correction.

Object of the study: peptic ulcer associated with Нelicobacter pylori. Purpose of the study: To optimize the diagnosis of peptic ulcer associated with HP with the account of immune homeostasis and functional morphology of gastric MM for working out the ade-quate treatment. Methods of research: General clinical, the study of cellular and humoral immunity, factor of nonspecific organism protection in the blood serum of gastric MM. To verify the diagnosis of PU endoscopy, x-ray study, morphological investigation were used. Theoretical and practical results: The necessity to use endoscopy, immunological, morphological study to diagnose PU associated with HP was substantiated. The study of cellular and humoral immunity as well as the factors of nonspecific organism protection in the blood serum and gastric MM of PU patients was proved to promoter improved diagnosis of PU. Correlation between immune homeostasis disorders, local immunity and HP contamination was established. Direct correlation between the degree of HP contamination and the level of neutral and acid mucopolysaccharides in the gastric MM was revealed. New ap-proaches to HP associated ulcer treatment including ad-ministration of the drugs producing positive effect on the immune system, namely Immunal, were worked out and tested. Optimal daily and course doses of the drug were suggested. The necessity to use the suggested complex pathogenetic therapy for PU was substantiated. Novelty: Disturnabces of cellular and humoral immunity as well as factors of nonspecific organism protection which can stimulate autoimmune reactions, aggravate pathological process and promote prolonged ulcer healing were revealed in PU patients. Immunomorphological and histological study of the bioptates showed that PU associated with HP there are disturbances of the local immunity: decreased activity of the first and second line of the immunomorphological protection, decreased amount of gluco-saminoglycans in the MM. Increase of the revealed changes severity with the elevation of the degree of the MM infection was proven. Correlation between the degree of HP infection and CD3, CD4, T-active lymphocyte amount in the blood serum, glycosaminoglycans in the MM, lymphocyte and immunoglobulin level in the blood serum was established. Optimum scheme of PU treatment with the consideration of the revealed disturbances was worked out; its efficacy was deter-mined. A new pathogenetic substantiation of Immunal use in PU with immune dysfunction treatment was given. Degree of introduction: The findings of the research are used in the work of therapy departments of City Clinical Hospital No. 7 and 8 (Kharkiv), gastroenterology department of City Clinical Hospital No. 2, are used in the teaching process of departemnt of therapy and clinical pharmacology of Kharkiv Medical Academy for Post-graduate Training.
