Shkolnik V. Influence of angiotensin-converting enzyme inhibitors on cytokines and endothelin-1 activity in patients with an acute myocardial infarction.

. Object of the research: Acute myocardial infarction. Purpose of the research: To improve the efficacy of treatment for acute myocardial infarction. Methods of the research: immunoenzyme essay for determining endothelin-1, 3'-, 5'-cyclic guanosine monophosphate, antiinflammatory cytokines (interleukin-1?, interleukin-6), MV fraction of creatine-phosphokinase (CFK). Practical significance of the obtained findings: The obtained findings promote optimization of diagnosis and therapy for AMI. Dynamics of AC content and factors of endothelial activity (ET-1, cGMP) in AMI patients was shown. The necessity of dose-dependent therapy with ACEI aimed at limiting necrosis zone and myocardium damage, prevention of development of complications of the disease and improvement of the clinical course and prognosis was substantiated pathogenetically. Novelty: For the first time, correlation between ET-1, cGMP, IL-1?, IL-6 was established in the patients with acute myocardial infarction depending on the course of thedisease. The use of IL-6 as an early diagnostic and prognostic marker of complication development was suggested. New mechanisms of ACEI effect from the perspective of correction of immune inflammation and neurohumoral indices, which prevent complication development, were studied. Dose-dependent effect of enalapril maleate on the function of endothelium and AC activity was shown. Degree of introduction: The findings of the research were introduced into the teaching process of therapy department of Kharkiv State medical University, to the work of reanimation and intensive therapy department, infarction department of Institute of Therapy (Academy of Medical Science of Ukraine), cardiology and reanimation department of hospital No. 27, infarction department of hospital No. 25, cardiology department of hospital No. 28 (Kharkiv), infarction department of hospital for urgent aid No. 4 (Kharkiv).
