Object of the study: nephrogenic hypertension, nephropaty. Purpose of the study: diagnostics optimization of cardio-vascular complications and increase of efficiency of treatment on patients with nephropaty on the basis of studying features of metabolism nitric oxide and left ventricular remodeling at patients with nephrogenic hypertension. Methods of research: general clinical and instrumental methods, photoelectrocolorimentry and chromatographic methods, left ventricular echocardiography. Theoretical and practical results: It is offered the approach to an estimation of metabolism nitric oxide with definition of concentration of substratum for synthesis nitric oxide - L-arginin, stable oxidized forms nitric oxide - nitrites and nitrates, and deposited form nitric oxide - nitrosothiol. Definition in dynamics of treatment of concentration of nitrosothiol, nitrites and their ratio can be used for an estimation of efficiency of pharmacotherapy. It is shown, that efficiency of influence on endothelial dysfunction and left ventricular remodeling at patients with nephrogenic hypertension depends on duration of treatment. Novelty: It is established, that at patients with chronic glomerulonephritis prevails excentric type of left ventricular hypertrophy, at patients with chronic pyelonephritis and arterial hypertension most distributes normal type of left ventricular geometry. Prevalence of concentric type of left ventricular hypertrophy at patients with chronic glomerulonephritis big in comparison with patients with chronic pyelonephritis, and patients with arterial hypertension. It is established, that at nephrogenic hypertension there is reorganization in system L-arginin - nitric oxide. At arterial hypertension and a chronic pyelonephritis on a background of an exhaustion of a physiological substratum for synthesis nitric oxide - L-arginin the contents of products of its oxidation (nitrites and nitrates) is reduced, and the parameter of deposition free nitric oxide - nitrosothiol exceeds a level of group of the control, over it at patients with a chronic pyelonephritis its level maximal. Patients with chronic glomerulonephritis are characterized by an exhaustion of all parts of this system as substratum for synthesis nitric oxide - L-arginin, the oxidized products nitric oxide, and a parameter of deposition free nitric oxide - nitrosothiol. ACE inhibitors are most effective at patients with a chronic pyelonephritis and arterial hypertension concerning return development of left ventricular hypertrophy, that proves to be true reduc-tion of index of left ventricular mass and quantities of patients with concentric type of left ventricular hypertrophy in 3 months of treatment.
