Essay

The main idea of the dissertation is optimization of treatment of acute coronary syndrome without segment ST elevation (unstable angina and non-Q-wave myocardial infarction) in patients with arterial hypertension by the differential application of beta-adrenoblockers with vasodilating properties, considering peculiarities of violations of 24-huor blood pressure profile, endothelium function and cardiohemodynamics. The presence of interrelations between parameters of 24-huor blood pressure profile, endothelium function and cardiohemodynamics was revealed. It was found, that carvedilol and nebivolol prevail metoprolol in ability to improve parameters of left ventricle diastolic function, 24-huor blood pressure profile, functional stage of endothelium, whereas more expressed negative chronotropic effect was typical for metoprolol. It was established the peculiarity of carvedilol to improve pump and contractile function of left ventricle in patients with non-Q-wave myocardial infarction and arterial hypertension.
