Lykhman V. Experimental-clinical substantiation of complex surgical treatment of patients with big and giant postoperative ventral hernias

The dissertation is devoted to the surgical treatment of extensive postoperative ventral hernias and development of complex ethiopathogenetic approach treatment of patients with big and giant postoperative ventral hernias. In the basis of work there principles of individualized selection of method of plasty of abdominal wall and prevention of postoperative complication. In experiment on animals it was proved possibility of commissural process prevention between intestine and implant by film-protector. The developed and introduced in praxis algorithm of treatment-diagnostic tactics, surgical methods by alloplasty and methods of complication prevention by the surgical treatment of patients with big and giant postoperative ventral hernias. Using of proposed complex method in treatment of basic group patients in comparison with another group allowed to improve results in the nearest and distant periods after operation: guantity of pyo-septyc complications of postoperative wound lowered from 6,7 to 3,2%,duration of lymphorrhea was descreased in 2,4 times, complications from the side of cardiopulmonary system lowered from 7,8 to 3,2% and guantity of reccurances decreased to 3,3%.
