Essay

The paper is devoted to the problems of increase of treatment effectiveness of the children sick with bronchial asthma on the basis of study of pathogenetic role of leukotrienes and nitric oxide in connection with indices of immunity and phagocytosis, function of external respiration. Comprehensive approach to evaluation of severity and prognosis for childhood bronchial asthma based on systematized anamnestic, clinical data, indices of specific and non-specific protection factors, and also mediators of chronic allergic inflammation was developed, which has contributed to obtaining more specific information on pathogenetic role of the latter and having varied approach to indication of basic therapy. The patients were registered to have an increase of the level of cysLT and NO2, and its degree directly depended on severity and acuity of pathologic process. Connection between immune and nonimmune mechanisms of disease development was found and theoretically grounded. Diagnostic value of the complex data of clinical (number of attacks a week, number of night attacks a week, number of days without asthma a month, obstructive syndrome intensity), general clinical research (level of monocytes, eosinophils in peripheral blood, ESR), biochemical indices (albumen in acute inflammation phase) and mediators of allergic inflammation (LT and NO) for estimation of severity of clinical course and prophylaxis of childhood bronchial asthma to grow progressively worse is emphasized. Necessity of using suggested markers and mediators of allergic inflammation as specific criteria of severity and acuity of chronic inflammatory process in case of bronchial asthma is shown. Usage of montelukast in complex basic antiinflammatory therapy of childhood bronchial asthma is proved.
