Оbject of the research: renoparenchymatous hypertension. Purpose of the research: improvement of the diagnostic of the morpho–functional reconstruction of the myocardium in the patients with renal arterial hypertension and elaboration of the recommendations for those treatment. Methods of the research: common clinical, biochemical, radio–immuno assay, immuno–enzyme assay, mathematical methods of figure processing. Practical importance of the received results – Diagnostic and prognostic value of the definition of the geometric model of the left ventricle in the patients with renal arterial hypertension was shone. It was proved, that the definition of the diastolic dysfunction is important for the diagnostics of the heart failure. The criterions of the use an ACE inhibitor Enalapril and calcium antagonist Dilthiasem in the patients with renal arterial hypertension were worked out. Scientific novelty of the received results: Peculiarities of the development of the infringement of the contractile ability of a myocardium in the patients with renal arterial hypertension were firstly revealed. We are analysed the peculiarities of the remodulation of the left ventricle, the connection between type of the remodulation and change of systolic and diastolic function. We firstly determined the influence of the endothelial dysfunction on the type of the heart remodulation, on the systolic and the diastolic function of the level ventricle. We proved the complex evaluation of the influence of the treatment by an ACE inhibitor Enalapril and calcium antagonist Dilthiasem on structural and functional properties of the left ventricle and level of endotelial dysfunction markers. Degree of introduction: The research results are introduced into the teaching of the chair of therapy and nephrology of the KMAPO and chair of hospital therapy of the Kharkiv state medical university; the research results are realized in the curricula of the therapy departments of the Region students hospital and of the 26–th clinical city hospital, Valky central district hospital.
