Object of the research: functional state of cardiovascular system in children operated for congenital heart diseases. Purpose of the research: the improvement of dispensary observation of children operated for congenital heart diseases in the remote period after a surgical correction on the basis of the advanced study of the left heart ventricle systolic and diastolic function. Methods of the research: general clinical, instrumental. Theoretical and practical findings: The relevance of dynamic administration of echocardiography-, Doppler echocardiographical study in children after a surgical correction was proved. The necessity of the systolic and diastolic function left heart ventricle indices study in order to improve the diagnostic, preventive and treatment qualities of heart failure was grounded. The contribution of the left heart ventricle systolic and diastolic dysfunction to the hemodynamic impairment in the remote postoperative period was investigated. The identification of heart failure varieties on the basis of echocardiographic criteria which were taken in the dynamics of observation can increase the quality of early diagnostics and treatment of chronic heart failure. The suggested prognosis tables can be used as the basis for the formation of dynamic observation groups depending on the risk of heart failure development. On the basis of received data the program for improvement of dispensary observation of children after a surgical correction for congenital heart diseases was developed. Novelty: For the first time the specific features of clinical and morphofunctional state of cardiovascular system in children with congenital heart diseases in the remote period after a surgical correction on the basis of Doppler echocardiographic indices were investigated. The contribution of systolic and diastolic left heart ventricle systolic and diastolic dysfunction to the hemodynamic impairment in congenital heart diseases after a surgical correction was identified. The relevance of dynamic administrationof echocardiography-, Doppler echocardiographical study in children operated for congenital heart diseases was scientifically grounded. The analyses of complications after a surgical correction for congenital heart diseases in the remote period of observation was performed. For the first time were shown early reclinical manifestations and prognostic criteria of progression and risk of heart failure development in children after a surgical correction for congenital heart diseases in the remote period of observation. A new method of heart failure variant diagnostics in children on the basis of Doppler echocardiographic indices received in the dynamics of observation was developed (declaration patent for the invention №70259А). The problem of dispensary observation of children operated for congenital heart diseases received further development. Degree of introduction: The results of the study are used in the practical work of the District Pediatric Cardiological Center, which was organized on the basis of Kharkov Regional Clinical Hospital, Simferopol Clinical Hospital №2, Donetsc Central Hospital №6, Dnepropetrovsk Pediatric Clinical Hospital №2, pediatric department of Dvuretchnsk, Bogodukhov, Sakhnovchina central district hospitals, which is confirmed by the respective administration acts. The results are used in the teaching process at pediatrics department of Kharkiv State Medical University. Sphere of application: Medicine, pediatrics.
