Mechanisms of development and significance of acute fluid collections in pancreatitis were studied and defined. The role of acute fluid collections in pancreatitis as reservoirs of toxic pancreatic enzymes and products of cellular degradation of cellular spaces and as source of intoxication was proved. It was found that autolysis and pancreatic, parapancreatic and retroperitoneal fatty tissue necrobiosis causes the development of acute fluid collections. Their development is caused by local and system inflammatory response. The necessity of severity scores (APACHE II) and dynamic ultrasound diagnostics and computed tomography in pancreatitis for definition of early surgical tactics was substantiated. Thorough analysis of treatment of patients with acute fluid collections in pancreatitis allowed theoretically substantiate diagnostic and treatment algorithms and to change surgical tactics in this pathology.
