Essay

It was proved that in elderly patients with acute Q-wave myocardial infarction the early, since the first day, administration of ACE inhibitor spirapril 6 mg/day in patients without concomitant renal or hepatic insufficiency is safe and so well tolerated as "low" dose therapy (3 mg/day). Positive impact on systemic inflammation and endothelial function was established. Significant positive dynamics of the left ventricle systolic function parameters by the 28th day of spirapril inclusion was seen only in case of anterior myocardial infarction and use of the full therapeutic dose. It is absent at "low" dose and posterior localization regardless of the dose.
