Changes in the regional hemodynamics and microcirculation were studied before and following the surgery in immediate and remote terms. Based on a comparative analysis of the results obtained in 456 patients with various alopecia types of cicatrical and androgen-dependent genesis, it was shown that local plastic interventions allowed positive immediate results to be obtained in 30 % patients with an area of replaced defects up to 25 cm2. The methods of controlled dermotension and extender plasty were capable of replacing alopecia areas up to 150 cm2; however, good aesthetic results were only observed in 45 to 75% cases in the remote period. The method of microfollicular hair transplantation, with plastic capabilities similar to those of the controlled dermotension and extender plasty methods, ensured good remote results in a higher number of cases. A therapeutic algorithm was developed for treatment of patients with cicatrical and androgen-dependent alopecia with an account for the microcirculation status.
