Dubrov S. Prevention and treatment for ventilator-associated pneumonia

. Dissertation is based on treatment of 467 patients and it is devoted to the questions of improvement of results of treatment patients with severe combined trauma which based on modification of prevention and treatment of ventilator-associated pneumonia. Proceeding from the results of the implemented comparative analysis it was found out that methods of care of patients introduced within the latest few years requiring long-term mechanical ventilation allowed achivement of decrease of frequency of VAP progression for 28.2% and lethality of this category of patients for 17.2%. Based upon the prospective research of influence of terms of tracheostomy timing fulfillment it was discovered that early tracheostomy (within 5 days prior to mechanical ventilation implementation) results in decrease of frequency of VAP progression. Definition of effectiveness of hourly endotracheal insertion of antibiotic solution in accordance with sensibility of microflora extracted from lower respiratory tract allows frequency of VAP and absolute risk of mortality of patients needing long-term respiratory support being reduced for 14,2%.
