Kuchyn I. The prophylaxis and treatment of respiratory failure in patients with Pulmonary Contusion

. Dissertation is based on treatment of 109 patients and it is devoted to the questions of improvement of results of treat-ment of victims with severe Pulmonary Contusion by the ap-plication of respiratory support which is based on vicissitude of the pathomorphology of contusion damage to the practical medicine. It involves a recession from routine use of the posi-tive end-expiratory pressure and application of ventilation in prone and sit-in positions. Ventilation in prone and sit-in posi-tions decreased intrapulmonary shunt and improved arterial blood oxygenation in patients with severe respiratory failure. The safety of use of this methods was proved. Application of ventilation in prone and sit-in position allowed reduce of VAP-incidents, diminish of Respiratory and ICU-stay Days in patients with Pulmonary Contusion. The implantation of con-temporary methods of the respiratory support allowed de-crease the common death-rate up to 11,3% and pulmonary causal mortality up to 3,23% in patients with severe Pulmo-nary Contusion.
