Polulyach I. The clinics, diagnostics and surgical treatment of the atypical forms of the choledocholithiasis.

The object-the atypical forms of the choledocholitiasis; the purpose-improve results of surgical treatment of patients with atypical forms of the choledocholitiasis by perfection of diagnostic algorithm, early diagnostics of choledocholitiasis and application of new methods of surgical treatment; methods-clinical, laboratory, radial, instrumental, statistical; novelty - the reasons of the atypical forms of the choledocholitiasis are first set and their patogenetical classification is improved; new algorithms of diagnostic search for the given category of patients are grounded; the role of the vegetative nervous system in pathogeny and clinical flow the atypical forms of the choledocholitiasis is studied; correlations between biochemical violations of blood, clinical forms of vegetative disfunction and variants of flow of postoperative period are first set; the value of dynamic interphase tensiometry whey of blood and bile in the prognosis of flow of disease and control of efficiency of treatment is proved; new technical receptions of operations is developed; quality of life at patients in a remote period is studied; results- to reduce quantity of postoperative complications from 19,8% to 8,9%, lethality - on the 2%, terms of treatment on the 7,4 days, money expenditures and labour expenditures in 2,4 times; applied - in medical practice, in an educational process; field- medicine.
