Dudukina S. Being of mechanisms of adaptations and choice of method of the anesthetic providing of operations in carotids

Dissertation is devoted the questions of experimental ground and introduction in practice of new method of the anaesthetic providing of carotid endarterectomy with the use of thiopental sodium consonant with intraoperative infusion of perftoran for patients with distsirkulatory encephalopaty of atherosclerotic, what carried sharp violations of cerebral circulation of blood, on the basis of findings in relation to neurological status, state of metabolism of brain, realization of mechanisms of adaptations and cognitive functions of TSNS. It is proved in an experiment that most favorably an ischemia flows at the use of perftoran, as on a background thiopental so to propofol anesthesia, but the optimum method of defenses of cerebrum vide an ischemia is the compatible use of tihpental sodium and perftoran. The use of the offered method in a clinic resulted in an improvement quality of treatment of patients, diminishing of frequency of peryoperative of neurological complications and more high-quality renewal of cognitive functions of TSNS. It is proved that the use of propofol for the anaesthetic providing of carotid endarterectomy must be limited and careful.
