On the basis of clinical inspection of 126 patients with acute serous pyelonephritis (ASP) the analysis of efficiency of therapy of pre and after conducting directed (insideorgan) electrophoresis antibiotic on a kidney area in terms of 7 and 14 days from the beginning of therapy. Into the І group the cases of unobstructive ASP were included, into the ІІ group - obstructive ASP, into ІІІ group - ASP on basis of urolithiasis, into IV - cases of chronic pyelonephritis in an active phase. The control was made on 30 patients with ASP who got classic medical treatment.The conducted analysis showed that during 2 weeks from the beginning of medical treatment there were marked positive changes in the functional condition of kidneys: levels of kreatinine , urea, the indexes of ionogram were stable or got better in all groups. The state of microbal landscape of urine indicated on more rapid elimination of microflora in basic groups, unlike the control, where at 1/3 cases pathogenic microflora of urine waskept. Improvement of immunomethabolic homeostasis during medical treatment in main groups was expressed in more powerful and rapid renewal unlike controlled group. So, the levels of malonic dialdehyde and dionic conjugant in biosubstratum diminished considerably on a background of activation of the AOS factors - superoxiddismutases and catalases in the І, ІІ and ІІІ groups, unlike the controlled group. Immunological factors in groups with directed intraorganic electrophoresis got better, due to reduction of deficit of suppressor link, increase of immunoregulyatory index, declining the CIK and PIF levels, which in the controlled group was not reliably expressed. Energization of renal blood circulation, with the use of directed , according to data of dynamic kidney scintiscanning was expressed in reduction of time of peak of T max and duration T1
