Essay

The work is dedicated to solving the urgent task of urology - improving the quality of specialized care for patients with renal cell cancer, taking into account prognostic trends in the prevalence of the pathology and the structure of its surgical treatment, using the proposed model of early diagnosis and the features of a personalized approach when expanding the indications for organ-sparing treatment. Based on the results of the conducted research, the nature of the trend changes in the incidence of renal cell cancer, prevalence and mortality from this pathology, as well as the characteristics of specialized surgical treatment in this cohort of patients, was determined, which made it possible to establish the epidemiological trend of the prognosis. Molecular genetic markers were studied to create an effective test system for early non-invasive diagnosis of renal cell cancer in 50 patients with kidney cancer in comparison with a control group of 15 healthy individuals with the identification of the most statistically significant indicators (concentration and integrity index of cfDNA, hypermethylation of RASSF1A and GPX3 genes). The basis for identifying and confirming the need to expand the indications for organ-sparing treatment in clinical stages T1b-T2 renal cell cancer was the data of a comparative analysis of the oncological and functional results of 167 patients who underwent surgical treatment - nephrectomy or resection of a kidney with a tumor. The need for an individualized approach in each case of diagnosing kidney cancer in the early stages has been confirmed, taking into account such factors as the location of the tumor, the presence/absence of chronic kidney disease (including compensated forms of CKD, stages I-II) and the presence of concomitant pathology, in addition to size tumors.
