The dissertation is dedicated to the topical question of thoracic surgery - the increase of effectiveness of surgical treatment of lung-diseased patients with pneumonectomy application on the basis of optimization of post-pneumonectomy syndrome course in the long-term postoperative period. Principal problematic aspects of clinic-functional state and patients' life quality after pneumonectomy, constituting the post-pneumonectomy syndrome, having as the main problem mediastinal herniation (diagnosticated in 81,1 % cases), were singled out on the basis of complete census of 132 patients. Preventive measures were developed to reduce the influence of post-pneumonectomy syndrome, decreasing the nascence of mediastinal herniation in 24,5 % cases, high-riding diaphragm in 10,1 % cases and esophagus' dislocation in 4,3 %.
