Essay

Object of the study: arterial hypertension. Purpose of research: improvement of renal and heart demage diagnostics on hypertension due to microalbuminuria and factors which adjust an exchange of extracellular matrix collagen studying. Methods of research: clinical, biochemical, immunoenzyme and instrumental methods were used. Practical value of the received results: obtained data have allowed to develop and improve early diagnostics of superfluous extracellular matrix collagen accumulation of kidneys and heart on hypertension. Incriasing of the transforming growth factor-?1 serum concentration is the feature of hypertensive renal damage forming and microalbuminuria development in future. Estimation of tissure ingibitor metalloproteinasis-1 serum level incriasing on hypertension is a ealy feature of renal and heart collagen accumulation changes. It is recommended to use definition of microalbuminuria in daily urine sick on hypertension not only as early attribute of hypertensive renal damage and left ventricular hypertrophy development, but also as attribute of infringement of balance between the transforming growth factor-?1 and tissure ingibitor metalloproteinasis-1 on hypertension. Novelty: the role of transforming growth factor-?1, proenzyme of metalloproteinase-1 and tissue ingibitor of metalloproteinases-1 in renal function damage and left ventricular changes forming on hypertension was estimated. It is revealed differences in concentration of factors which were studied, and in albumin excretion level on hypertension compare with healthy people. Differensis in serum concentration factors which were studied, depending on albumin excration rate were estimated. Correlation between transforming growth factor-?1, proenzyme of metalloproteinase-1 and tissue ingibitor of metalloproteinases-1 serum concentrations and ehocardiografy left ventricular myocardium parametrs changes were estimated. The sequence of increase of each of circulating factors which were studied, depending on expressiveness ofstructurallyfunctional changes of a left ventricular myocardium is established. It is shown, that in patients on hypertension with left ventricular mass index equal or more than 128 g/m? and end-diastolic diameter index equal or more than 2,61 sm/m? there are changes in proenzyme of metalloproteinase-1 level already compare with healthy persons. These data are put in a basis of a new way of intersticial myocardial fibrosis diagnostics (the patent of Ukraine for the useful invention № 10763, 2005). The role of the transforming growth factor-?1 in deranged renal formation, namely, in microalbuminuria development was proved. It is established also, that increase of the circulating transforming growth factor-?1 occurs in patients hypertrophic types of left ventricular remodeling. Degree of introduction: results of research are introduced in practical work of medical establishments of Kharkov and region (therapeutic departments of city clinical hospital № 11, Balakleaand Bogoduchov the central regional hospitals), central regional hospitals therapeutic departments of Gluhov and cardiological department of Shostka.
