118 patients with chronic obstructive pulmonary disease have been assessed clinically, as well as by means of electrocardiography, spirometry and echocardiography. Forty-two (46%) of patients appeared to have left ventricular (LV) diastolic dysfunction. Positive correlations were found between pulmonary artery mean arterial pressure and the degree of left ventricular diastolic dysfunction. According to multivariable analysis, pulmonary hypertension appeared to be an independent predictor of left ventricular diastolic dysfunction. The degree of respiratory failure correlated positively with LV filling indices: positively with IVRT and negatively with E/A ratio. The use of Candesar was associated with significant improvement of LV filling indices, decrease in dyspnea severity. Transformation of LV impaired relaxation into normal pattern was more frequently observed in Candesar group. A tendency towards aggravation of LV filling disturbances was seen in a group taking standard therapy only.
