Essay

The dissertation summarizes the results of anesthesiological supply of bariathric interventions in 122 patients with morbid obesity. Based on the results of clinical data, laboratory tests and instrumental evaluation, we have established, that patients, who suffer from excessive body weight, have dysfunction of cardiovascular system, breath and metabolic disturbances. Influence of different methods of anesthesiological supply on the haemodynamics, ventilation and metabolism data as also as on postoperative complications was investigated. We have established the necessity of usage of preoperative oxygen saturation in different body positions to evaluate the efficacy of medical physical training and optimal body position in postoperative period for prophylaxis of pulmonary complications. Advantages of regional methods of anesthesia and percutaneous electric stimulation of nerves in this group of patients in postoperative period for avoiding breathe depression and prolonged artificial pulmonary ventilation in early postoperative period were proved. Usage of the proposed tactics of anesthesiological supply made it possible to decrease the risk of haemodynamic, pulmonary and thromboembolic complications, mortality and to decrease the duration of patients treatment.
