Object of the study: insufficient biological readiness (BR) to labor, pathological preliminary period (PPP). Purpose of the study: to reduce the incidence of anomalies of labor activity by the study of etiopathogenetic mechanisms of disturbances of prelabor preparation (PP) in the system mother-placenta-fetus with development of therapeutic-preventive measures aimed at improvement of prelabor preparation of pregnant with high risk of their development as well as treatment of PPP. Methods of the study: clinical-laboratory, clinical instrumental, immunoenzyme, histomorphological, immunohistochemical and statistical methods. Practical significance: A new method of PP was worked out. The use of tissue therapy with cryopreserved placenta tissue (CPT) during PP extends the range of the methods of treatment of PPP, preparation of uterine cervix (UC) and prevention of labor activity anomalies (LAA) in contemporary obstetrics, which allows to decrease maternal and perinatal morbidity and mortality, to reduce the duration of hospital stay, to reduce the risk of polypragmasy and negative influence of drugs on the organism of the mather and fetus. Novelty of the obtained results: For the first time, complex analysis of the changes in local cytokine profile of the pregnant with low BR to labor in PPP, the state of the fetus and its stress-limiting system (в-endorphine) as well as intracellular cooperation in the placenta was carried out. New approaches to PP were worked out, the efficacy of tissue therapy with CPT application for stimulation of adequate prelabor transformations was substantiated. The analysis of clinical course of preparation period, transformation of hormonal and cytokine profile under the influence of CPT was carried out. Main pathogenetic links which normalize the process of UC maturation and prepare the fetoplacental complex to the labor under the influence of CPT were determined. The indications, optimal protocols of tissue therapy with CPT for correction of the states connected with insufficientprelabor rearrangement as well as of LAA prevention were worked out. For the first time, it was scientifically proved that the use of tissue therapy with CPT in PP is effective for the adequate maturation of the UC followed by initiation of labor activity. Introduction: PP method was introduced into the work of maternity departments of maternity hospitals No. 1, 2, 5, 7 (Kharkiv), Maternity Hospital with Neonatology In-patient Department (Kharkiv), Izium Central District Hospital. The findings of the research were used in the patent (No. 49672 -A "A method of prelabor preparation"). Sphere of application: medicine, obstetrics and gynecology.
