Futujma J. Indications and contraindications for gastric ischemic segment wedge resection during ulcerous disease surgical treatment – early and late result prognosis

Dissertation deals with the development of the indications and contraindications for organ-safe gastric ischemic segment wedge resection during the treatment of patients with ulcerous disease of type I. The results of surgical treatment of patients with ulcerous disease of type I according to Johnson classification have been analysed. The dynamics of the local blood circulation reduction of mucous membrane of gastric periulcerous zone has been shown depending on ulcer localisation. On the basis of the morphologic, morphometric and immunomorphologic investigation, the correlation of small and minute artery morphometric changes have been indicated in the ulcer region with the indices of local blood circulation, local immune reactions and the difficulty of dysplastic changes in gastric mucous epithelium. The indications and conditions for organ-safe gastric ischemic segment wedge resection of patients with ulcerous disease of type I have been analysed and determined. The algorithm for gastric ischemic segment wedge resection has been proposed depending on ulcer localisation and morphologic changes in the ulcerous infiltration region.
