Essay

Object - patients, operated concerning bullous emphysema of lungs. Purpose - to improvement of quality of anaesthetic aid at operations concerning bullous emphysema of lungs due to the choice of the most optimum methods of general anaesthesia from point of stability of vegetative homoeostasis in a perioperative period. Methods - clinical, laboratory, instrumental, statistical. Novelty - first set that at operations concerning bullous emphysema of lungs indexes of heart rate variability are more sensible markers of adequacy of anaesthesia, than traditionally in-use indexes of central hemodynamics, oxygen transport, gases of blood and acid-basic state. First developed and inculcated methods of optimization of general anaesthesia at patients, operated concerning bullous emphysema of lungs, from point of stability of the vegetative adjusting in an intraoperative and nearest postoperative period. Results - it is proven that application of technique of an pre-emptive analgesia for patients, operated concerning bullous emphysema of lungs, allows to eliminate taped at propofol-based anesthesia and halothane-based anesthesia during operations in occasion of bullous emphysema of lungs a undesirable strain of regulatory-adaptive systems in patients at some stages of the operating period.
