Object of the study: Duodenal ulcer disease combined with mitral valve prolapse. Purpose of the study: To describe the mechanisms of impairment of the protective mucous barrier by means of determining blood melatonin level as one of the regulators of central and regional hemodynamics in students with DUD combined with MVP and to develop the methods of differentiated pathogenetic correction. Methods of the investigation: general clinical, immunoenzyme, instrumental, endoscopy, x-ray, morphological investigations. Theoretical and practical results: The necessity to use endo-scopic, immunoenzyme, ultrasound, morphological investigations for diagnosis of DUD confined with MVP was substantiated. As-sociation of the degree of melatonin level reduction, worsening the parameters of central and regional hemodynamics, diameter of the abdominal trunk and the level of neutral and acid mucopoly-saccharides in the MM of the gastroduodenal zone in patients with DUD was established. A new approach to treatment of DUDassociated with MVP including Melaxen, which positively influences the parameters of hemodynamics in optimal and course doses, was suggested and tested. Novelty: It was established that the presence of MVP combined with DUD is characterized by more pronounced hemodynamic disorders, reduction of the heart index and regional circulation of the gastroduodenal zone. A distinct correlation of the peculiarities of the clinical picture of the disease, the presence of MVP, degree of hemodynamic disorders and melatonin level was revealed. One of the mechanisms causing impairment of protective mucous bar-rier via the disorders in the processes in the central and regional hemodynamics in DUD was shown to be the impairment of melatonin secretion. An optimum protocol of treatment of the students with DUD accompanied by MVP was worked out with the consideration of the obtained findings; its efficacy was proved. Degree of introduction: The findings of the research were introduced to the work of therapy department of City Clinical Hospital No. 7 (Kharkiv), gastroenterology department of Regional Students’ Hospital (Kharkiv) and are used in the teaching process at Department of Therapy and Clinical Pharmacology of Kharkiv Medical Academy for Post-graduate Training.
