Essay

The thesis deals with perfection of therapy effectiveness for patients with cervical intraepithelial neoplasms I and II degrees of different course severity as well as with prediction of disease development. The author shows the most probable risk factors of papillomavirus-induced cervical intraepithelial neoplasms I and II degrees are the following: early beginning of sexual intercourses, numerous sex partners, childbearing, aborts, menstrual cycle disturbance, use of oral contraceptive drugs, and smoking. Several the most dangerous markers of cervical intraepithelial neoplasms are shown including the high frequency of stabile koilocytic atypy, degree of increased epithelial mitotic activity detected as pathologic mitoses, and presence of high carcinogenic papillomaviruses (types 16 and 18) in neck of uterus samples. The course of disease is demonstrated to be accompanied by disturbed values of cellular and humoral immunity indices caused by the advanced deep suppression of gamma- and alfa-interferonsynthesis, disturbance of tumor necrosis factor-alfa production as well as of soluble receptors type I for this cytokine (factor 55). The use of different interferon preparations combined with cryodestruction increases the effectiveness of patient treatment with intraepithelial neoplasms; as a result, the normalization of interferon process synthesis is seen as well as intensification of other indices of the organism's immune reactivity.
