Dissertation is devoted to the problem of diagnostic, to the questions concerning the treatment policy of endovascular treatment different form of unstable angina. Estimation of the results of endovascular interventions allowed to define indications and contraindications to them, to extend the contingent of IHD patients with unstable angina, for which endovascular interventions are accompanied the promoted risk of development of complications. The optimum methods of coronary stenting are developed. On the basis of study of changes of ECG parameters, estimation of preoperative coronarography data, the analysis of specialities of the intraoperative stage the algorithm of diagnostic and treatment of IHD patients with unstable angina is developed. The features of the anaesthesiological providing of endovascular interventions are certain. The considerable decline of frequency of postoperative complications is attained. The study of long term results testifies to high stability of effect of both methods (endovascular and surgical) of treatment IHD patients with unstable angina. Survival in long terms after an operation for certain did not differ and was to 3 year a supervision 93,6% for patients operated with coronary stenting and 88,1% for patients operated with coronary artery bypass grafting. The result of application of early endovascular and surgical interventions, partial revascularization and methods of direct stenting was become by the considerable decline of hospital mortality which for period from 2000 for 2004 was 1,6%.
