3. Object of the study: Thyrotoxic cardiomyopathy. Purpose of the study: To improve the results of treatment of patients with thyrotoxic cardiomyopathy by means of working out pathogenetically substantiated methods of correction of disorders in oxidation homeostasis and cytokine production. Methods of research: Clinical, instrumental, biochemical, immunological, statistical. Theoretical and practical results: A pathogenetically substantiated scheme of complex treatment of thyrotoxic cardiomyopathy with the use of vita-melatonin and mindronate was worked out and tested. Application of the suggested protocol allows improving the results of treatment of the patients with thyrotoxic cardiomyopathy complicated with cardiac failure. Novelty: For the first time, the parameters of systolic and diastolic function of the left ventricle were compared in patients with uncomplicated and complicated with cardiomyopathy thyrotoxicosis. Initial signs of diastolic dysfunction at increased parameters of the systolic function of the left ventricle in patients with thyrotoxicosis without cardiac signs were revealed. Progress of diastolic dysfunction with the increase of thyrotoxicosis duration and development of cardiac signs was revealed. Dependence of LP and AOS processes dysbalance and duration of thyrotoxicosis and cardiac failure was revealed. High positive correlation between the degree of anti-inflammatory cytokine activation and disorders of oxidation homeostasis was revealed. It was proved that the degree of left ventricle diastolic dys-function progresses with the development of disorders in oxidation homeostasis and increase of concentration of anti-inflammatory cytokines. For the first time vita-melatonin and mildronat were used in the complex treatment. Positive influence of the antioxidant and metabolic therapy in patients with thyrotoxic cardiomyopathy was proved. Degree of introduction: The suggested treatment protocol was introduced to the work of Kharkiv City Hospital for Urgent Medical Aid, Railroad Hospital, regional Cardiology Hospital (Chernovtsy) and teaching process of department of clinical pharmacology and phytotherapy of Institute of Advanced training for Pharmacy Specialists of National Pharmacy University, department of internal diseases of Kharkiv State medical University.
