Essay

The dissertation is devoted to conducting of scientific substantiation of optimal use of known transurethral low-invasive methods of surgical interventions in the patients with benignant hyperplasia of prostate with high operative risk and the implementation of new methods of surgery directed to restoring of independent urination, increase of treatment efficiency and life quality amongst these patients. The research was conducted in 178 males with the presence of benignant prostate hyperplasia with age 54-85 years old. There was demonstrated that the use of standard palliative surgery in acute retention of urine is not enough effective in the patients with benignant hyperplasia of prostate with high operative risk. The use of traditional invasive methods of surgery in the patients with benignant hyperplasia of prostate with high operative risk results in the occurrence of the significant amount of the complications and even lethal exits in early and late post-operative period. There was approved thatthe use of low invasive surgical interventions in the patients with benignant hyperplasia of prostate with high operative risk - incision of the urinary bladder's cervix and posterior urethra if the prostate size is less than 50 cm and incision of the urinary bladder's cervix and prostate with the simultaneous partial transurethral electroresection of prostate if the size of hyperplasic prostate is more than 60 cm3 are characterised with high effectiveness. There was confirmed that transurethral surgery is effective method of treatment of the patients with the prostate hyperplasia complicated with prostate abscess. This surgical intervention is directed to cutting abscess with simultaneous resection of hyperplasic tissue of prostate with electrical knife. There was first time approved that transurethral injection of ethylic spirit into the hyperplasic tissue of prostate and following transurethral electroresection is highly effective method of complex treatment if prostate size is more than 80 cm3.
