Dissertation is dedicated to the advance of efficacy of treatment of patients with acute purulent pyelonephritis based on the evaluation of conservative, minimally invasive and surgical treatment results with use of clinical, laboratory, immunological methods of investigation and ultrasonic dopplerography. The best results of treatment were observed in localized forms of acute purulent pyelonephritis. It was determined that a background factors of chronization were the age above 60 years, immunological insufficiency, the obstructive character of pathological process, diffuse, extended forms of disease. It was argued that ultrasonic dopplerography can objectivize unilateral or bilateral character of pathologic process and evaluate a structural and functional status of each kidney. The indications for the particular treatment methods were validated. The algorithm of dynamic observation after acute purulent pyelonephritis considering age, immune status and pathological process character was developed.
