Essay

The dissertation thesis deals with the effects of hydrochlorothiazide and ACE inhibitors combined therapy in patients with essential hypertension with varying arterial intima-media thickness.Thickness of the carotid arterial intima-media in patients with essential hypertension was found to be associated with an aggravation of disturbances in the ABP diurnal rhythm, increase of flow volume asymmetry between contralateral main cerebral arteries, and to correlate with an increase of left ventricle myocardium mass index. In some groups of patients with essential hypertension captopril/hydro-chlorothiazide and lizinopril/hydrochlorothiazide combinations were shown to produce prompt and significant hypotensive effect owing to which the first administration may result in some cases in a decrease of blood flow in main cerebral arteries and occurrence of clinical signs of cerebral ischemia. Fosinopril in combination with hydrochlorothiazide (F+H) provide steady but gradual (less than 8% per hour) antihypertensive effect, and cerebral blood flow volume remained unchanged .
