Semchyshyn R. Differentiated Therapy of Gestational Hypertension.

The thesis daelth with the improvement of the therapy of gestational hypertension, closely connected with the gestational transformation vessel resistance of the placental bed of uterus, by means of the pathogenetic correction of the primary blood flow disturbances in the spiral arteries. The performed investigations showed that in 34 -38% of cases with gestational hypertension, there were disturbances in the utero - placental vessel formation. The primary disturbances of the cytotrophoblast invasion into the wall of the spiral arteries were present as well. Dopplerometric investigation of the uteroplasental blood flow, in case of disturbances of the vascular angiogenesis of the placental bed of the uterus, reveals the presence of the high blood flow resistance in the uterine artery. Moreover, the indices of the vascular resistance (S/D, PI and RI) in the spiral arteries exceed the standards by 25 - 32%. The use of the antihypoxanth actovegin and the calcium antagonist -verapamil in GH with high indices of the vascular resistance in the spiral arteries permits to form the low-resistant blood flow in the uterine artery basin and has a positive effect on the central hemodynamics in pregnant.
