Essay

Object of research: malignant neoplasms of a thyroid gland which have arisen at the population of Ukraine during after accident on the Chernobyl. The purpose of research: to learn results of different treatment methods of malignant tumours of a thyroid gland, the prognostic importance of risk factors. To develop a two-stage thyroidectomy in case of preoperation and intraoperation conclusion „ follicular neoplasia " and on this basis to increase efficiency of treatment such patients. Methods and apparatus: clinical, surgical, prognostic and statistical. Theoretical and practical results: was research the remote results of radical treatment of high differentiated thyroid carcinoma which have arisen at the population of Ukraine in period after Chernobyl accident. At comparison of different methods of treatment 1808 patients is confirmed, that in a postaccidental period a thyroidectomy is the basic method for treatment of thyroid cancer with subsequent radioiodtherapy. The system of prognostic analysis of relapse and method of surgical treatment of follicles formations of thyroid was developed. The offered operative method is used for the preoperated and inrooperated conclusion "follicular neoplasia" and subsequent establishment of malignant character of tumour (patent №3967 from a 15.12.04. "Method of surgical treatment thyroid follicular neoplasia"). Novelty: it is shown, that during after the Chernobyl accident the morbidity of a thyroid cancer is certain enlarged in comparison with morbidity of a cancer by other localization. In connection with problems of operative volume choice in cases of absence definite conclusions for cytohistological researches ("follicular neoplasia") was developed the method of operative treatment of these tumours, and also system of prognostical analysis, which allows to estimate development of relapse. The level of approach: Results can be used in treatment of patients with neoplasms of a thyroid gland Field of application: endocrinology, surgery.
