Zelena I. Clinic-pathogenetic substantiation of treatment for patients with the chronic gastritis type B in combination with iron deficiency anemia.

Object of the study: pathogenetic links of chronic gastritis type B (CGB) in combination with iron deficiency anemia (IDA). Purpose of the study: to improve treatment of patient with CGB in combination with IDA on the grounds of determination clinic-patogenetic particularities of their duration. Methods of the investigation: clinical (questioning, physical examination); clinical laboratory (indicators of clinical analysis of blood, urine and gastric secretion); titrometric (determination of concentration of ascorbic acid (AA) in gastric juice); colorimetric (determination of concentration of iron in serum); biochemistry (ureas-test); immunofermental (determination of level of ferritin in serum, immunofermental express test on determination of antibody to Н.рylori); instrumental (videoesophagogastroduodenoscopy with biopsy of gastric mucous membrane (GMM), intragastric pH-metria, electrocardiography); morphological and histological (valuation of condition of GMM, discover of Н.рylori); statistical.Theoretical and practical results: results of dissertation widening knowledge of particularities of duration CGB in combination with IDA. It was proved that IDA has negative influence on duration of CGB, which revealed in clinical and morphological particularities. It was uncovered relation between level of AA in gastric juice and expression of changes in main indices of iron metabolism which are depend on the level of contamination GMM Н.рylori, pH of gastric juice and level of atrophy of GMM. It was proposed to use level of AA in gastric juice as a prognosis criterion of development of iron deficiency in patients with CGB. Novelty: for the first time was uncovered relation between level of contamination GMM Н.рylori and level of AA in gastric juice. It was shown that AA of gastric juice may be prognosis factor of development of iron deficiency in patients with CGB. For the first time were revealed criterions of eradication therapy in patients with CGB in combination with IDA. It was demonstrated new pathogeneticsubstantiation of the expediency of using of iron-contain medicine which hold supplementary AA during the complex treatment of patients with CGB and IDA. Degree of introduction: results of dissertation are used in treatment of patients with CGB in combination with IDA in gastroenterological department of Institute of therapy named by L.T.Malaya, department of internal medicine of hospital №28 Kharkiv, department of internal medicine of Kharkiv outpatient clinic №8, department of internal medicine of Kharkiv outpatient clinic №20 and department of internal medicine of Cherkass regional hospital which are confirmed by acts of introduction.
