Startseva M. The effect of a transrectal urinary derivation on function of the gastro-intestinal tract

The research work is based on a study and scientific ground the management of after operation treatment and rehabilitation of patients with bladder disease, who underwent cystectomy with transrectal urinary diversion. Research is based on the results of complex investigation and treatment 70 patients after transrectal urinary diversion. The results of investigation gastro-intestinal tract show a change of the functional parameters of rectosigmoid part of colon: decrease the intraluminal pressure (base and maximum) and increase the volume in the reservoir area. There is provide a rationale of adequate reservoir volume and algorithm of it creative. This scheme prevents appearance the pathologic reflux in patients with transrectal urinary diversion. Prolonged contact sigmoid colon mucosa with urine elicited in early inflammatory phase, followed by an atrophic phase after year. It was established that the rate of early and late postoperative complications was higher in patients with history of previous treatment for main disease (organ-preserving operations, irradiations, chemotherapy). There is propose the algorithm of diagnostic and prophylactic clinically significant metabolic acidosis by the basis of investigating metabolic disorder.
