Essay

Thesis is devoted to research of role of ischemic-reperfusive liver injury at surgical treatment of patients with obstructive jaundice of benign genesis. The time of beginning of this syndrome, peak of its action and duration of injury at different preoperative levels of bilirubin in blood serum have been determined, and its dependence on duration of jaundice has been investigated. A main damage factor and sources of its appearance have been found out, and the pathogenetic motivated medicamental correction has been offered. It has been established the activity of hepatocytolisys at miniinvasive and laparotomic operations, the purpose of which was decompression of bile ducts. The risk of appearance of acute liver failure in a postdecompressive period in patients with obstructive jaundice has been established. On this basis it has been offered the algorithm of choice of method of operation, directed on prevention of appearance of hepatargia in early postoperative period.
