Object - the secondary cataract in children after congenital cataract removal. Purpose - to increase the efficacy of surgical treatment of children with congenital cataracts. Methods - visometry, biomicroscopy, ophthalmoscopy, tonometry, ultrasound scan, electronic microscopy, statistical. There were established that the frequency of secondary cataract development is 21,3% in a year after operation and 43,5% in 2 years of follow-up. Secondary cataract at 7-15 y/o children develops in 2,4 times more rare (23,4%), than in younger children (56,4%). There was established that "false secondary cataract" develops on pseudophakic eyes at the zone of removed posterior capsule and it consists of pathologically changed lens epithelial cells and vitreous body connective tissue fibres. At first time the relative efficacy of secondary cataract intraoperative prophylaxis during congenital cataract removal with foldable IOL implantation was established: posterior capsule opening does not influence on secondary cataract frequency (55,4%) comparatively with the preserving of posterior capsule (48,9%), its combination with anterior vitrectomy increases secondary cataract frequency to 28% but provokes the development of other postoperative complications. The results have been adopted at the Filatov Institute and at Odessa city clinical hospital №4. The sphere of application: medicine, ophthalmology.
