Essay

The results of examination of patients with acute respiratory failure that is caused by the closed thoracic trauma are summarized in the thesis. We showed that the main reasons of respiratory abnormalities are pain syndrome and primary lung injury because of contusion. We proved that in the case of thoracic trauma the best antinociceptive potential and stabilization influence on the oxygenation has regional analgesia in contrast to narcosis. However, the viewed methods of regional analgesia do not prevent the existence and spreading of destructive changes in lung parenchyma, because of contusion. We proved the efficiency of the method of prolonged potentiated subpleural blockade on the base of optimal analgesia, as it prevents the alterative processes in contusion area. As the result of using of the method of prolonged potentiated subpleural blockade we obtained the decreasing amount of infectious respiratory complications and lethality.
