The dissertation is devoted to the important and actual problem of improvement in the treatment of patients with decompensated diabetes mellitus at the state of precoma and coma based on study of cerebral bioenergetics and dynamics of the cerebral metabolism in condition of traditional intensive care treatment with involvement of drug - reamberin. As a result of this investigation, the particular qualitative changes of cerebral bioenergetics (oxygen balance and hydrocabonate exchange) depending on heaviness of clinical condition and accompanying pathology were discovered. We have found that patients with decompensated diabetes mellitus without heavy changes in life important organs and systems have compensated bioenergetic hypoxia of cerebral neurons, patients with accompanying nephropathy - decompensated reversible or partially reversible hypoxia, patients in terminal state - decompensated irreversible bioenergetic hypoxia of cerebral cells. Involvement of pathogenetic therapy in combination with energetic corrector reamberin in a dose of 100mg/kg per day was followed by clear positive dynamic of oxygen balance of cerebral hydrocabonate exchange with improved clinical neurological symptoms and more faster withdrawal from decompensated condition with recovering consciousness.
