Essay

Dissertation is devoted to the study of metabolic peculiarities, state of coronary blood circulation, cardiohemodynamics findings, function of endothelium, systemic inflammation in dependence with body mass index in patients with ischemic heart disease (IHD): stable angina pectoris of tension, II-III function classes and impact of beta-adrenoblockers on these findings. On the basis of a complex investigation it was determined, that in case when body mass index increases there increases a number of patients with insulin-resistance, levels of total cholesterin (TC), low density lipoproteins (LDLP), triglycerides (TG), C-reactive protein, factor of tumor-apha necrosis, number of affected coronary vessels, amount of a final-diastolic size (FDS), final-systolic size (FSS), final-diastolic volume (FDV), final-systolic volume (FSV), but the level of high density lipoproteins (HDLP) decreases as compared with a control group. In all patients examined there was revealed dysfunction of endothelium. Correlation link between body mass index and insulin-resistance, LDLP, HDLP, TG, endothelium dysfunction, C-reactive protein, factor of tumor-alpha necrosis were determined. After carried out therapy with metoprolol use, there increases level of insulin, dyslipidemia manifestations, findings of C-reactive protein and factor of tumor-alpha necrosis. Carvedilol positively acts upon findings of carbohydrate and lipid exchanges, cardiohemodynamics, function of endothelium, systemic inflammation.
