Kukhto A. Substantiation of the peculiarities of Crohn’s diseases surgical treatment

Object – Surgical treatment of Crohn’s disease of the small and large intestine, purpose – the improvement of the results of surgical treatment of Crohn’s disease by means of perfecting the surgical tactics and techniques of operative treatment. The method’s are clinical, laboratory, immunologic, endoscopic, pathomorphologic, functional, statistical. Novelty for the first time the objective criteria of necessity of surgical treatment in case of relative indications of an operation for patients with Crohn’s disease have been developed. For the first time a differentiated surgical tactics depending on the process location, the degree of the inflammatory changes development in the affected intestine wall, a clinical course of the process, an immunologic status of patients in accordance with the scheduled follow-up care has been grounded scientifically. For the first time the connection of general clinic, biochemical blood indices, TNF-?, IL8 with exacerbation and remission of the main disease that is confirmed by the findings of the pathomorphological study of operational and biopsy material has been studied clinically. For the first time on the bases of the obtained finding’s, a neuronetwork model of prognosing Crohn’s disease exacerbation in the postoperative period and interstage treatment has been created; Results - The use of the offered surgical tactic in the explored group by comparison to a control one allowed to reduce lethality to 4,4±2,5%, to form a reservoir and recover intestinal continuity at 92,3±7,4% in the explored group; it has been itroduced into medical practice and studying process; sphere – medicine.
