Essay

Object of the study: dyshormonal diseases of the mammary glands (DDMG). Purpose of the study: an increase in the treatment efficacy of DDMG by the determination of the prognostic significance and pathophysiological role of disturbances in the reproductive system resulting from genital endometriosis, uterine leiomyoma, chronic inflammation of the genitals with regard to the influence of the treatment on target organs in the reproductive system. Methods of the study: questioning; clinical, instrumental, radioimmune, roentgenological, medical-statistical methods. Practical significance: on the basis of clinical-statistical studies: a conceptual proposition was made about the prognosis of risk factors for the appearance of DDMG in female patients with the reproductive system dysfunction (in genital endometriosis, uterine leiomyoma, chronic inflammation of the genitals); a diagnostic algorithm was improved for concomitant pathology of the genitals and target organs (mammary and thyroid glands); the main principles of the medical tactics, organization of rehabilitative measures at the interdisciplinary level in patients with mastopathy and reproductive system disturbances of different genesis were established. A positive effect of differential hormone therapy, systemic enzyme therapy, naturopathic remedies, local use of gestagen, homeopathic remedies aimed at restoration of endocrine homeostasis was proved; differential indices were determined for the formation of risk groups for the appearance of DDMG at the age aspect, prevention of systemic disturbances in patients with gynaecological pathology; the necessity of observing patients with DDMG by physicians-gynaecologists was grounded and proved. Novelty of the obtained results: for the first time, the psychoemotional characteristic data and hormonal homeostasis indices were analysed in a complex and compared with the state of the mammary glands in women, who had infertility and/or incompetent pregnancy in their case history as a result of uterine leiomyoma, genital endometriosis, and chronic inflammation of the genitals, in order to assess the safety of the reproductive system reserves and prognosticate rehabilitation of the child-bearing function. Clinical-laboratory-diagnostic grounds for the efficacy of the medical tactics, consisting of medicamental and nonmedicamental methods of influence, in mastopathy patients were perfected. For the first time, conceptual relationships were es-tablished for DDMG, which have pathogenetic unity with common gynaecological pathology (uterine leiomyoma, genital endometriosis, chronic inflammation of the genitals), taking into consideration aggravated gynaecological, perinatal anamnesis and premorbid background, thereby making it possible to increase the clinical efficacy of the suggested medical-rehabilitative programme. Degree of introduction: the improved diagnostic algorithm and ways for preventing DDMG in female patients with uterine leiomyoma, genital endometriosis, and chronic inflammation of the genitals were introduced into the practice of female dispensaries of Vilnogirsk Central Town Hospital, Verkhnedniprovsk Central District Hospital, Krinichansk Central District Hospital, Dniprodzerzhinsk City Hospital No. 9 of the Dnipropetrovsk Region. Sphere of application: medicine, obstetrics and gynаecology.
