Essay

Object - the deep, subfascial soft tissue defects of the lower leg; the purpose - the improvement of the results of lower leg deep subfascial defects surgical treatment on the base of elaborated methods of plasty with the local vascularised flaps; methods - clinical, laboratory, planimetrical, rontgenological, ultrasonic, angiographic, topographical anatomic, morphometric, mathematical modelling, statistical; novelty - For the first time on the base of angioarchitectonics investigation with the use mathematic modelling the differential approach for the planning and forming of the local vascularised flaps is grounded. For the first time the new method of lower leg soft tissue deep defects plasty is developed on the base of local fasciocutaneous flaps with the consideration of lower leg angioarchitectonics.The differential indications for choosing the method of lower leg subfascial defects plasty according to their lacalisation, area and depth are grounded; results - The developed methods of lower leg soft tissue subfascial defects plasty with the use of local vascularised flaps allowed to achieve the closure of the defect in 98,0% of all cases, to lower the rate of the unsatisfactory results and complications in 5,9 times and to shorten the number and stages of plastic surgeries. The use of polycomponent vascularised flaps with the axial bloodflow allowed to improve the long term results: the rate of good results rised from 57,6 % to 73,6 %, is introduced - into medical practice, in educational process; area - medicine.
