Essay

Object: malignant thyroid tumors. Aim: To ivestigate oxyphilic cell cancer (OC) peculiarities in comparison with follicular (FC) and papillary (PC) thyroid cancer. To determine the main prognostic factors for OC and work out the rational algorithm for treatment and diagnosis of tumors among such kind of patients. Methods and equipment: clinical, ultrasound, radiological, morphological; sonograph “Sonoline-Sienna”, microscope “Leica”, computer tomograph «E.Cam». Theoretical and practical results: according to the results, oxyphilic cell cancer differs from PC and FC what include older age, higher level of extrathyroidal invasion, initial regional metastases and late metastases development, insensitivity to I131-diagnosis and treatment. The results of the investigation allowed to work out recommendations for improving the treatment and observation of the patients with OC. Radiologic method what was introduced for OC reccurences and metastases allows to detect them on early stage and without thyroid hormones withdrawal. Novelty: At first time the analysis of peculiarities of clinical behavior, diagnostic and treatment methods of OC was done in comparison with other types of differentiated thyroid cancer. It was proved that patients with OC need of different approach to treatment and observation. The radionuclide method for metastases and recurences detection was worked out. It based on using double scintigraphy with 99 mТc-pertechnetate and 99 mТc-МIBI (patent №9221). At first time the algorithm for treatment and diagnosis of patients with OC was woked out. Introduction: the results allowed to introduce to medical practice of endocrinologists and oncologists technology of diagnostic of I131-negative OC metastases and were offered for using for the lead through of educational process among medical students and doctors.
