Essay

The thesis is devoted to use of new surgical method of hemostasis and sealing the vessels of medium and large diameter with the help of automatic program of electric tissue welding.Experimentally established optimal parameters of electric tissue welding impulse for reliable occlusion of arterial and venous vessels 2-12 mm in diameter became a basis for development of vessel welding automatic program. The strength of vessel welding connections, obtained with the use of this program were not less then 500 mm Hg (66,65 кРа). In clinic the method of vessel welding has been used for performance of operative investigations in 137 patients. It has been established, that vessel welding in surgical treatment of varicose desease allows the reduction of operation duration on 23-35% and avoiding the use of foreign body. Application of developed and clinically introduced welding way of vein occlusion more proximally the apex of thrombus is a reliable method for prophylaxis of pulmonary thromboembolism. It allows toavoid complications caused by implantation of cava-filter and does not worsen hemodynamic conditions of blood outflow from the lower extremity. It was proved, that use of method of vessel welding in performing accesses to main vessels and carrying out limb amputation provides a reliable and fast hemostasis, that causes depression of wound complications frequency, decreases blood loss level on 50-55%, reduces operation time on 30-35%.
